Blepharoplasty

Blepharoplasty - Eyelid Rejuvenation Surgery

Blepharoplasty (eyelid surgery) refreshes tired, aged eyes by removing excess skin, fat, and puffiness from upper and/or lower eyelids. Eyes are the first thing people notice - droopy eyelids make you appear exhausted, sad, or older than you feel, even when you are well-rested.

This subtle yet transformative procedure restores youthful, alert eyes without changing your natural eye shape. Results are dramatic yet natural-looking - people notice you look refreshed, but cannot tell you had surgery.

What Blepharoplasty Corrects

Upper Eyelids:

  • • Hooded, drooping excess skin
  • • Impaired peripheral vision
  • • Puffy upper lid fullness (fat pockets)
  • • Tired, angry resting expression

Lower Eyelids:

  • • Under-eye bags (fat herniation)
  • • Loose, wrinkled skin
  • • Dark circles / hollowing
  • • Festoons (malar mounds)

Upper vs Lower Blepharoplasty

Upper Blepharoplasty

Problem: Excess skin creates heavy, hooded appearance. Lashes disappear, eyeliner impossible to apply, makeup creases.

Solution: Remove crescent of excess skin + fat through hidden incision in natural eyelid crease. Scar becomes invisible white line matching crease.

Anesthesia: Local with sedation or general | Time: 45-60 minutes | Recovery: 7-10 days

Lower Blepharoplasty

Problem: Fat pockets bulge forward creating bags. Loose skin wrinkles. Tear trough hollowing casts shadows (dark circles).

Solution: Two approaches -

  • Transcutaneous: Incision just below lashes. Remove/reposition fat, tighten skin. Scar fades to invisible line.
  • Transconjunctival: Incision inside eyelid (no external scar). Remove fat only. Best for younger patients with good skin.

Anesthesia: Local with sedation or general | Time: 1-2 hours | Recovery: 10-14 days

The Procedure

Step 1: Precise Marking

While sitting upright (gravity shows true excess), surgeon marks exactly where skin/fat will be removed. Precision is critical - removing too much causes complications.

Step 2: Anesthesia

Local anesthesia with IV sedation (twilight sleep) is most common. You are comfortable but not fully asleep. Some patients prefer general anesthesia.

Step 3: Incisions & Removal

Upper: Incision in natural crease. Excess skin + fat removed. Lower: Incision below lashes or inside lid. Fat removed/repositioned, skin tightened if needed.

Step 4: Delicate Closure

Ultra-fine sutures (6-0 or 7-0 thickness) close incisions. Meticulous technique ensures scars heal flat and imperceptible.

Recovery Timeline

Day 1-3

Peak Swelling: Eyes puffy, bruised (looks like black eyes). Cannot wear contacts. Vision slightly blurry from ointment. Ice packs every 2 hours. Sleep elevated 45 degrees.

Day 4-7

Sutures removed day 5-7. Swelling 50% resolved. Bruising fading from purple to yellow/green. Can cover with makeup starting day 7. Return to work if comfortable.

Week 2

90% healed. Minimal swelling remains (only you notice). Bruising gone. Can wear contacts again. Resume normal activities except exercise.

Week 4-6

Resume all activities including strenuous exercise. Incision lines still pink - massage with Vitamin E oil to soften.

Month 3-6

Final Result: Scars fade to thin white lines matching natural creases - completely invisible. Eyes look bright, refreshed, youthful.

Often Combined Procedures

Blepharoplasty addresses eyelids only. For complete upper face rejuvenation, combine with:

  • Brow Lift: Raises droopy brows pulling down on upper lids
  • Facelift: Tightens lower face, creating overall harmony
  • Fat Grafting: Fills hollow tear troughs under eyes
  • Laser Resurfacing: Smooths wrinkles, improves skin texture
  • Botox: Relaxes crows feet wrinkles around eyes

Am I a Candidate?

Good candidates:

  • Age 35+ with droopy eyelids or under-eye bags
  • Healthy, non-smoker, realistic expectations
  • No active eye diseases (glaucoma, dry eye, retinal issues)
  • Stable thyroid function (thyroid affects eye appearance)

Not suitable if: Untreated dry eye (surgery worsens it), Certain autoimmune diseases affecting healing
