Gynecomastia Surgery - Male Breast Reduction
Gynecomastia surgery eliminates enlarged male breasts (often called "man boobs" or "moobs") by removing excess glandular tissue and fat to create a flat, masculine chest contour. This condition affects 40-60% of men at some point in life and can cause significant embarrassment, avoiding activities like swimming or going shirtless.
The procedure is life-changing - most men report dramatic improvement in confidence and quality of life. Results are permanent (if weight remains stable), allowing you to wear fitted shirts and swim without self-consciousness.
What Causes Gynecomastia?
- Hormonal Imbalance: Estrogen/testosterone ratio disrupted (most common - 60% of cases)
- Medications: Steroids, antidepressants, heart meds, chemotherapy
- Puberty: Temporary hormonal surge (resolves spontaneously in 75% of teens by age 18)
- Aging: Testosterone decreases after age 50
- Weight Gain: Excess body fat converts to estrogen
- Idiopathic: No identifiable cause (25% of cases)
Gynecomastia vs Pseudogynecomastia
True Gynecomastia (Glandular)
- • Firm rubbery tissue under nipple
- • Feels like a disc or pebble (1-2 inches)
- • Cannot be reduced by diet/exercise
- • May be tender/sensitive
- • Requires surgical gland excision
Pseudogynecomastia (Fat Only)
- • Soft, fatty tissue (no hard disc)
- • Entire chest area enlarged
- • CAN improve with weight loss
- • Not tender
- • Liposuction alone may suffice
Note: Most men have MIXED gynecomastia (gland + fat) requiring both liposuction and gland excision.
Grading System
Grade I: Minimal Enlargement
Small button of glandular tissue under areola. No skin excess.
Treatment: Gland excision through small incision at areola edge
Grade II: Moderate Enlargement
Breast tissue extends beyond areola. Minimal skin excess.
Treatment: Gland excision + liposuction
Grade III: Marked Enlargement
Significant breast tissue with moderate skin excess.
Treatment: Gland excision + lipo + skin tightening
Grade IV: Severe (Feminine Breasts)
Large breasts with substantial hanging skin excess.
Treatment: Skin excision required (results in visible scars)
The Procedure
Step 1: Liposuction
Small cannulas remove excess fatty tissue around gland and chest perimeter. Creates initial flattening.
Step 2: Gland Excision
Incision at lower edge of areola (semicircular cut blends with color change). Rubbery gland tissue carefully removed with scissors/scalpel. Leave thin layer to prevent crater/nipple inversion.
Step 3: Nipple Reduction (if needed)
If nipples puffy/enlarged, excess tissue trimmed from behind nipple to create flat, masculine appearance.
Step 4: Closure
Dissolving sutures close incision. Compression vest applied (wear 4 weeks 24/7). Surgery takes 1-2 hours.
Recovery Timeline
Compression Vest On: Wear 24/7. Moderate swelling, bruising. Some drainage. Mild discomfort (feels tight). Can shower day 2-3.
Return to desk work week 1-2. Swelling 50% resolved. Chest looks flatter but still swollen. Avoid upper body exercise.
Resume all exercise including chest workouts. Swelling 90% gone. Can go shirtless! Scars pink but fading.
Final Result: Chest completely flat and masculine. Scars matured to thin white lines at areola edge (barely visible). Wear fitted shirts confidently!
Important Pre-Surgery Requirements
Before surgery, your doctor will order:
- Hormone Blood Test: Check testosterone, estrogen, thyroid, prolactin levels
- Mammogram/Ultrasound: Rule out breast cancer (rare in men but possible)
- Medication Review: Stop steroids/supplements that may have caused gynecomastia
- Weight Stabilization: Reach goal weight before surgery (fat regrowth if gain weight after)
Red Flag: If gynecomastia appeared suddenly or one-sided only, requires medical evaluation before considering surgery

Maintaining Results
Gynecomastia surgery is permanent - removed gland tissue cannot grow back. However:
- Maintain stable weight: Gaining 20+ lbs can cause fat accumulation in chest
- Avoid steroids: Anabolic steroids cause gynecomastia recurrence
- Manage hormones: If caused by medication, work with doctor on alternatives
- Build chest muscle: Strength training creates defined pecs enhancing flat chest appearance
